Spring 2015 Seeding Request Form

Any team registering to play in WYSL Spring must complete a "Seedings Request Form."

Please log in to your team page by finding your division and team on the Schedule and Standings page for Fall 2013 or Spring 2014 season below.

To fill out the "Seedings Request Form"you must be listed on the team page as one of the Team Contacts.  To get to the "Seedings Request Form" click on the "Team Log In" at top right of the page.

If you have forgotten your pin number you may request it and it will be emailed to you. 

When entering contact information, make sure you enter TWO DIFFERENT CONTACTS.

If you need to get back into your application AFTER you completed the above, please use this link:


In order for a team to log back into his/her application they will need to ask the system to send him/her a pin # and team #.  They will click the link that says, "If you don’t have a PIN# or TEAM#, or you’ve forgotten it, then click here to have it sent to you."  The system will send the team contact the pin # and team # for all teams that use the email address entered.  The contact can then modify the application, enter a roster (if needed), and pay (if he/she hasn’t already).


Clubs can now print ALL team applications at one time from the Club Directory.

Have a team login to their club page.  

  1. Click on Team Administration. 
  2. At the top of the page where it says "Spring 2014" or "Fall 2014" they will see a blue "P".  If they click on the P it will open up all of the team applications for that season for all of the teams.  
  3. Then they can just File > Print or Control P to print that screen.

US Club Certificate of Insurance - Multiple Facilities


US Club Instructions:
Fill out one form (see attached) for your club and one for each facility (or group of facilities if from the same organization, municipality or other certificate holder) that your club is utilizing, and send each form to AdminRegionA@usclubsoccer.org for processing. The certificates will then be sent to the e-mail address you provide.  Note that the e-mail address furnished should be a generic club e-mail address, as the same certificates will be generated to that address yearly.
